Different Form Types

"Online" Web Forms

If you can't complete the web form online and cannot email it to us either, then you can post it to us with any other required information.

Download and Save the Web Form

  1. Click on the form you require.
    1. The form should be displayed in your browser.
    2. You can either:
      1. Complete the fields and then print, or
      2. Leave the fields blank and manually fill them in, then
  2. Press the Print button at the bottom of the form and the Print dialogue is displayed
  3. You can then:
    1. print the form on your printer, or
    2. If you want to save a copy for yourself:
      1. Select that you want to Save the document as a PDF (do not print), and
      2. If necessary, select the file destination
      3. Save the file on your local machine
      4. Then open the file as a PDF and print it off to complete manually.

Standard Word Forms

The standard forms can be completed on word processor software (such as Word or Pages) or as PDF documents.  Once completed you can print and post them to us with any other required information.

Download and Save the Standard Word Form

  1. Click on the form you require.
    1. The form should be opened in Word /Pages or similar application. If it is, then,
  2. Complete the form and print it off (complete any remaining areas manually).

If the form does not open on your machine, it may be because you do not have compatible software.  If this happens, you can save the file using Print and Save as a PDF on your browser and then print and complete the information.

Send by Post

Once the form is complete you can post it us (except active employees of Non-TOCs, Network Rail and BTP - see below), attaching the form and any other required documents or information.

Please do not send original documents in the post as RST cannot guarantee their safe return and will not be liable if they are lost or destroyed.

In addition, RST are not liable for any loss or damage as a result of items sent to or from us being lost or delayed in the postal system. For the avoidance of doubt, this includes returned cards and passes that may affect future travel eligibility or charges - items lost in this way cannot be replaced by payment of a replacement fee and should be sent to us by "Signed For" delivery methods.

Active Employees of Non-TOCs, Network Rail and BTP

Please contact your usual travel facilities contact point as below:

  • Active non-TOC staff - forms MUST be sent to your usual travel facilities contact point. In most cases, this will be via your employer.
  • Network Rail staff - forms MUST go via the Safeguarded Helpdesk
  • BTP staff - forms MUST go via the HR Business Centre
