Notes On Completion of this Form

This form must enclose ongoing proof of unemployment to establish your eligibility to rail staff travel facilities. This includes, but is not limited to:
  • For HMRC Statement - the statement or letter must:
    • be dated within the last 3 months,
    • be on HMRC letterheaded paper,
    • confirm that you have no earned income for the last fiscal year or for any fiscal year where you have not previously provided proof of unemployment
      You will also be required to prove that you are currently unemployed.
  • For DWP Statement - the statement or letter must:
    • be dated within the last 3 months,
    • be on DWP (or similar) letterheaded paper,
    • confirm that you are currently unemployed,
    • confirm that you have been continuously unemployed for the last 12 months (or since the date of your last declaration, whichever is the earlier).
      You will also be required to prove unemployment for any period where you have not previously provided proof.
  • For P60U Document - the document must:
    • be dated for the last fiscal year,
    • be the official P60U issued to you.
      You will also be required to prove that you are currently unemployed.
  • For Doctor's Letter - the letter must:
    • be dated within the last 3 months,
    • be on the Doctor's or surgery's letterheaded paper,
    • confirm that, in their opinion, you are currently unable to work due to ill health,
    • confirm that, in their opinion, you have been continuously unable to work due to ill health for the last 12 months (or since the date of your last declaration, whichever is the earlier).
      You will also be required to prove you have been continuously unable to work due to ill health for any period where you have not previously provided proof.
      Note: if the Doctor confirms that owing to your condition and continuing ill health that in their opinion you will never be able to work again in any role or capacity, then you won’t need to provide annual proof in the future. Continued entitlement then may depend on the willingness of an employer to pay the requisite fees
RST may require further supporting documentation to establish eligibility.
